Athletic Forms
Please note that there is a $75 district participation fee for all athletics. This is per athlete, not per sport. This fee is due prior to participation in any sport. High school athletes will pay the bookkeeper at AHS and middle school athletes will pay the bookkeeper at their school.
Before returning forms to the coach, please make sure all forms are signed and dated
